A6000 Jessops Cashback

I bought an  A6000 in Jessops on 29 November 2017 in Glasgow - great deal as it had come with a £100 cash back.

I cannot after searching the Sony site find out how you claim the cashback.  Can someone send me a link to the form to claim the cash back.

Link here - hurry though, the promotion closes on 21 January!

Clarification - although purchases must be made on or before that date, cashback claims can be submitted up to midnight on 21 February.
