Ussd code running while calling

hi i get a problem when making a call.when i want to make a call it shows "USSD CODE IS RUNNING" with the call page on the screen.but it calls to that person and shows this message all the time until i press ok.what should i do?my phone has a problem or my card? and my android version is 4.1.please answer.it's emergency
