My charger has been fine since I got my phone a year ago, however today I plugged it into my PC to get some video footage and it kept disconnecting from windows, I realised that the charger was not connected even though I had done it. When I plugged it in, it was very loose in the port, and I'm really confused. Any help?

Just to be on the safe side, could you try with a different USB cable and see if that stays connected / your phone continues to charge properly?
If it is the same with a different cable, follow the suggestion that @Harza gave and try and see if there is any dirt or dust in the port that makes the cable not sit in place. I would not recommend using a needle but a toothpick should do the job!

I had the exact same problem. There's dust/dirt in the port, so the charger won't go in. Shut down the device, and dig out the dust carefully with a needle.
